Dental Visit
To book an appointment at Bayview Village Dental Specialists, you can contact us via phone, email, or simply fill in and submit this form. Please show up 15 minutes prior to your appointment to have enough time to complete any paper work required.
Your initial appointment will start with a consultation to determine your diagnosis and accordingly present you with your treatment options. Your dental specialist will discuss your options thoroughly with you and answer all your questions, until you can both agree on a treatment plan that both of you feel comfortable with. We will then provide you with an estimate and we’ll gladly submit it electronically to your insurance company. Then, we can finally schedule your treatment appointment with us.
Our highly knowledgeable and well-trained staff is always willing to assist you in any way possible. For your first dental visit they will need the following:
– Your referral slip (if you’re referred to us by another doctor)
– Any recent X-rays (if applicable)
– List of medication you’re currently taking
– Dental insurance information

We’ll gladly take care of your dental needs in the best way we can, while taking your financial situation into consideration. We’ll be happy to come up with a flexible payment plan that you feel comfortable with and that enables us to provide you with nothing less than our quality dental services.
For your convenience, we accept cash, debit, Visa, MasterCard and American Express. Patients are encouraged to get familiar with their dental plan benefits to know if their treatment will be covered or not. Even though, most dental plans offer assistance, they rarely cover the entire fee. To save you time and effort, we’ll be happy to submit your insurance claim electronically, including your treatment estimate.
